The Relationship between Changes in the Production of Tumor Necrosis Factor (TNF)-¥á, TNF-¥â, Soluble TNF Receptor 1 (sTNFR1), sTNFR2 by Whole Blood Cells and Changes in Bone Mineral Density after Estrogen-progestogen Therapy in Postmenopausal Women

Abstract
Objectives: To investigate the association between changes in the production of tumor necrosis factor (TNF)- ¥á, TNF-¥â, soluble TNF receptor 1 (sTNFR1) and sTNFR2 by whole blood cells (WBCs) and changes in bone mineral density (BMD) after estrogen-progestogen therapy (EPT) in postmenopausal women.

Methods: The production of TNF-¥á, TNF-¥â, sTNFR1 and sTNFR2 by lipopolysaccharide-stimulated WBCs, serum levels of bone alkaline phosphatase, carboxy-terminal C-telopeptide of type I collagen, parathyroid hormone, calcium, and phosphorus were measured in 67 postmenopausal Korean women. The BMD at lumbar spine and proximal femur was also determined. In all, 38 women had been treated with sequential EPT for 1 year.

Results: The basal production of TNF and sTNFR by WBCs was not associated with age, years since menopause, body mass index, bone turnover markers and the BMD at all skeletal sites. The basal TNF and sTNFR production was not different according to the state of BMD. After 6 months of EPT, the production of sTNFR2 by WBCs decreased significantly, whereas the production of TNF-¥á, TNF-¥â, sTNFR1 were unchanged. The 1 year-change in BMD after EPT did not show any relationship with the basal production of TNF and sTNFR by WBCs and their changes after 6 months of EPT.

Conclusion: After EPT, the changes in the production of TNF and sTNFR by WBCs were not associated with the changes of BMD, but sTNFR2 production by WBCs decreased significantly after EPT.
